Indemnity
This is to certify that I, , of NRIC/PP , in consideration of the acceptance of my entry, I, for my heirs, executors and administrators release and forever discharge SGA Golf Club Pte Ltd, Raffles Country Club, Transport Svcs, their sponsors, their appointed staff, officials or agents, all liabilities, claims, actions, damages, costs or expenses which I may have against them arising out of or in any way connected with my participation in this event, including traveling to or from this event, and including all injuries that may be suffered by me before, during or after this event.
I understand that this indemnity includes any claims based on negligence, action or inaction of any of the above parties.
I also declare that I have golfer's insurance. |
